We may contact your insurance provider to verify benefits and obtain any required authorizations. Verification of benefits is not a guarantee of payment or coverage. Coverage and payment are subject to your health plan’s terms, limitations, and exclusions at the time services are provided. Any services denied or not covered by your insurer may become your financial responsibility.

Insurance verification is a check of what your health plan reports for addiction treatment. At Legacy Healing LA, the admissions team contacts your insurance provider directly, confirms your behavioral health benefits, and sends you a written summary in plain language.
Verification gives you information to plan with. It is not an approval for treatment, and it does not replace the clinical evaluation that decides which level of care fits you. The sections below separate each part of the process so you can see who decides what.
Most verifications are completed the same day. If your plan requires prior authorization, insurer review adds time. Admissions reaches you by phone, using the number you gave us.
The insurance company and member ID let the team find your plan. The patient’s name and date of birth confirm the right person. The phone number is how admissions reaches you.
If you are verifying for someone else, the details are the same. Provide the patient’s insurance company, member ID, full name, and date of birth.
Keep the card nearby when you call or fill out the form. If you don’t have it, call us and the team will tell you what else may help.
Admissions checks whether your policy is active, which behavioral health benefits your plan lists, whether Legacy Healing LA is in network, what your deductible, copay, and coinsurance are, and whether your insurer requires prior authorization. The table shows what each result tells you and what it leaves open.
| Term | What It Tells You | What It Does Not Settle |
| Eligibility | Whether your plan is active on the date checked | Whether the plan will pay for a specific service |
| Benefits | What the plan reports for addiction treatment, plus deductible, copay, and coinsurance | What the insurer will approve for you personally |
| Network Status | Whether Legacy Healing LA is in or out of network with your plan | Whether your costs will match an estimate |
| Authorization | Whether the plan requires advance approval for treatment | Whether the insurer approves it |
| Medical-Necessity Review | The insurer’s check of your clinical information against its criteria | Whether the review comes out in your favor |
| Patient Responsibility | What you may owe after the plan pays, as an estimate | The exact amount, which depends on the services you receive |
| Final Payment | Nothing at this stage, because it happens after treatment | What the insurer pays on the claim |
Here is a hypothetical example. If your plan’s allowed amount for a service is $1,000 and your coinsurance is 20% after the deductible is met, you would owe $200 for that service. Your own plan will differ.
Questions worth asking admissions:
Verification is not a guarantee of coverage, payment, authorization, eligibility, or admission. A benefits check shows what your plan reports on the day it was checked. Several things can change that picture afterward:
Admission is a separate decision. Our clinical team decides whether a program fits your needs after an evaluation. Admission also depends on bed availability and financial arrangements being in place.
If you have a PPO plan, verification checks whether Legacy Healing LA is in network with your plan. If it isn’t, we check whether your plan includes out-of-network benefits for substance use treatment. Out-of-network does not automatically mean uncovered.
PPO plans often cover out-of-network care at a higher cost. That usually means a separate deductible and a higher coinsurance percentage. A hypothetical plan might charge 20% coinsurance in-network and 40% out-of-network, which changes your share considerably.
Coinsurance is calculated on the amount your plan allows for the service. If a facility’s charge is higher, the difference may be billed to you. Ask admissions for your estimated out-of-pocket total before you decide on an admission date. Our guide to PPO insurance for substance abuse treatment covers how this applies.
Legacy Healing LA offers self-pay options. People choose them when they have no insurance, when their plan has no usable benefits, or when they prefer not to use insurance.
Our financial team provides a cost breakdown before treatment begins. Paying privately changes how treatment is billed and removes the authorization step. It does not change the clinical evaluation, the level of care we recommend, or the practical steps of bed availability and travel.
Cost depends on your level of care, length of stay, and the services included. Our guide to what luxury rehab costs in Los Angeles explains the factors, and admissions can walk you through the numbers for your situation.
Verification is the beginning of admissions. After you receive your written summary, the usual order is:
Admission depends on clinical fit, an open bed, safe transport, and financial arrangements being in place. If any of these is pending, the date can move, and admissions will tell you what is unresolved. Read the full walkthrough in our article on what happens after rehab verifies your insurance, or learn about our travel assistance.
Call (800) 808-4570 at any hour if any of these apply:
If someone is in immediate danger, such as a suspected overdose, a seizure, or severe withdrawal, call 911 instead of waiting for verification.
Your privacy is our priority. Your information is kept private. Verification is confidential and creates no obligation to enter treatment. Read our privacy promise to see how Legacy Healing handles confidential information.

Payment of benefits is subject to all terms, conditions, limitations, and exclusions of the member’s contract at the time of service. If your health insurance company determines that a particular service is not reasonable and necessary, or that a particular service is not covered under your plan, your insurer will deny payment for that service, and it will become your responsibility.
